Charge Ready NY 2.0 Program Resources

 

All MUD and Workplace Equipment Owners that receive funding from the Charge Ready 2.0 program must send out the quarterly newsletters to their employees/tenants.

Equipment Owner Agreement [PDF] Link opens in new window - close new window to return to this page.: View the standard equipment owner contract for participation. You will sign this as part of the application process.

Installer Agreement [PDF] Link opens in new window - close new window to return to this page.: If an installer submits an application on behalf of the Equipment Owner, the installer and the Equipment Owner will need to sign this as part of the application process 

Site Host Agreement [PDF] Link opens in new window - close new window to return to this page.: Download this form, print it, and have the owner of the site where the charging station will be installed sign it. If you are the site owner and the equipment owner, sign this agreement to confirm. You will need to submit this signed document with the rebate application.

If you are the installer and the equipment owner, sign this agreement to confirm. You will need to submit this signed document with the rebate application.

Department of Ag and Markets EVSE Notification Form [PDF]: New York State Agriculture and Markets Law Section 182 requires Equipment Owners to notify the Department of Agriculture and Markets when new public electric vehicle charging stations are installed. An Agriculture & Markets EVSE Notification Report must be filled out and submitted by email or fax. Serial numbers for additional chargers should be entered in the Comments/Remarks box at the bottom of the form. Information is included on the Agriculture and Markets EVSE Notification Form. A copy of this form must be submitted with final documentation.
