Before filling out the form, confirm your eligibility for the SAMES program by verifying the following statements are true.

Affordable housing [PDF] is defined as buildings in which at least 25% of the units are, or are expected to be, occupied by households earning not more than 80% of the area or State median income, whichever is higher. Affordable housing documentation will be required before SAMES support is approved and dispersed.*
